XERF is a monopolar radiofrequency system from Cynosure Lutronic that combines two radiofrequencies — 2 MHz and 6.78 MHz — to heat tissue at more than one depth.
Most RF devices work at a single frequency, targeting one depth of tissue. XERF delivers two frequencies at once, which the manufacturer describes as depositing energy in the dermis (where collagen lives) and in deeper tissue, without incisions.
What that produces in practice is gradual tightening of existing collagen and new collagen over the months that follow, with little downtime. It is not a facelift and does not reproduce a surgical result.

Some patients notice tightening on the day, partly from swelling. Collagen remodelling continues over the following weeks and is reviewed at about three months. Published XERF studies have not followed patients beyond three months, so longer-term durability is not yet established. The realistic timeline.
Your face has multiple layers. Most non-invasive treatments act on the epidermis and upper dermis — the surface. Laxity, however, involves deeper tissue as well, including the SMAS (Superficial Muscular Aponeurotic System), the layer surgeons tighten in a facelift.
XERF uses two frequencies, 6.78 MHz and 2 MHz, delivered in sequence within a treatment. The manufacturer's design rationale is that the higher frequency deposits energy more superficially, in the dermis, and the lower frequency reaches deeper tissue. That rationale rests on laboratory and animal work; no published human trial has compared dual-frequency with single-frequency delivery, and a radiofrequency treatment is not equivalent to surgery. Many patients ask how XERF compares to HIFU (Ultherapy), Thermage, or other skin tightening options. Here is a straightforward comparison.

XERF received US FDA 510(k) clearance K251327 on 11 August 2025. The applicant was Lutronic Corporation, and the device was found substantially equivalent to the Thermage FLX (K170758).
The cleared indication reads: “XERF is indicated for use in dermatologic and general surgical procedures for electrocoagulation and hemostasis.” Skin tightening comes from that controlled heating of tissue.
A 510(k) clearance is not an FDA “approval”, and the submission relied on bench testing against the predicate device rather than clinical trials. It tells you the device has been reviewed by a regulator. It does not guarantee a result. How XERF and Thermage compare.
Common, and expected: redness and warmth for a few hours; mild swelling, which can last a day or two; skin that feels tender to the touch for a few days.
Uncommon: small raised marks from the treatment tip, which settle within days; temporary tingling or numbness.
What we have observed across more than 100 XERF treatments, including who tends not to be a good candidate.
Rare, but reported with monopolar radiofrequency: burns or blisters, and very occasionally a small area of fat loss that leaves a slight dip in the skin. Careful energy settings and continuous skin cooling are how these are avoided.
Message the clinic on WhatsApp if you notice blistering, a spreading red area, or pain that is getting worse rather than settling.
Absolute contraindications: pacemakers, implantable cardioverter-defibrillators or any implanted electrical device — monopolar radiofrequency passes current through tissue and can interfere with them. Pregnancy.
Requires assessment before treatment: metal implants or permanent fillers in the treatment area; active skin infection or inflammatory disease at the site; uncontrolled diabetes affecting wound healing; history of keloid scarring; isotretinoin within the last six months; current or recent malignancy; severe uncontrolled cardiac disease.
These questions are asked before every XERF treatment at Elea Clinic, by the dermatologist rather than a therapist. Some patients are advised against treatment. A provider who does not screen for implanted electrical devices before applying monopolar radiofrequency is not treating safely.

Is XERF the same as HIFU?
No. HIFU (High-Intensity Focused Ultrasound) uses sound energy at single focal points. XERF uses dual-frequency monopolar radiofrequency, delivering two frequencies, 6.78 and 2 MHz, which the manufacturer describes as reaching two tissue depths. Different technology, different mechanism, different experience.
Is XERF FDA approved?
XERF is FDA 510(k) cleared (K251327, 11 August 2025), which is a different process from FDA approval. The cleared indication is “dermatologic and general surgical procedures for electrocoagulation and hemostasis”. The clearance was based on equivalence to the Thermage FLX, not on clinical trials.
Is XERF painful?
Most patients describe a deep, warm sensation during each pulse. The handpiece cools the skin surface throughout, and Cynosure Lutronic states that treatment does not normally need numbing. dr. Jesslyn adjusts the energy to your tolerance during the session.
How many sessions will I need?
Many patients see visible improvement after a single session. The collagen remodelling process continues for 4–12 weeks, with peak results at approximately 3 months. A maintenance session every 6–12 months is typically recommended to sustain results.
What areas can XERF treat?
XERF is most commonly used for the lower face and jawline, cheeks, neck, and brow area. It is particularly effective for treating early to moderate skin laxity, jowling, and loss of facial contour. dr. Jesslyn will assess which areas are most appropriate for you.
Is there any downtime after XERF?
