Home · Neck Tightening in Bali: Why the Neck Needs It
Skin, fat, muscle and bone all shape a neck. Radiofrequency treats one of the four, which is why the first job is working out which one is bothering you.
Short answer. The neck ages in several distinct ways: loose skin, fat under the chin, vertical muscle bands, crepey surface texture, and the structure of the jaw and hyoid beneath. XERF and other radiofrequency treatments can improve mild-to-moderate skin laxity of the neck. They do not treat muscle bands or remove significant fat. Neck skin is thinner than facial skin and is treated at lower energy, and the thyroid region is avoided entirely.
Neck skin is thinner, has fewer oil glands and is often more sun-damaged than people realise. Beneath it lies the platysma, a thin sheet of muscle that can form visible vertical bands with age; below that sit fat, the jaw and the hyoid bone. Clinicians often use the Dedo classification to describe neck ageing (Pérez & Hohman, StatPearls).
| Dedo class | Main feature | Does radiofrequency help? |
|---|---|---|
| I | Normal, well-defined neck | Not needed |
| II | Skin laxity only | Yes — the best match for RF |
| III | Fat under the chin | Only partly; the fat itself needs another approach |
| IV | Platysmal banding | No. Bands are muscle: botulinum toxin or surgery |
| V | Retrognathia (recessed jaw or chin) | No. This is structural |
| VI | Low-lying hyoid | No. This is structural |
Most necks are a mixture, which is why “neck tightening” is rarely one treatment.
For skin laxity — class II, and the laxity component of mixed necks — monopolar radiofrequency can tighten skin and stimulate collagen. One early study treated 20 necks and 30 cheeks with mild-to-moderate laxity and reported significant improvement in most patients, while noting that longevity was not yet known (Alster & Tanzi 2004). A 2025 meta-analysis found no significant difference between HIFU and radiofrequency for the neck (Modena 2025), so either can be reasonable and the choice depends on the individual. XERF-specific neck data are very limited; the published XERF studies focus on the face and lower face.
As covered in the advanced XERF programme in Korea (Seoul article): the neck is treated at lower energy than the jawline; the thyroid and vocal-cord region is avoided entirely rather than simply treated more gently; and the under-chin area is often treated at a deeper setting as part of the lower-face pass, while the lower neck receives a lighter pass.
“The neck is where I most often change a patient’s plan after examining them. Someone may come in asking for neck tightening, but what bothers them in the mirror is two bands when they speak, or fat under the chin. Radiofrequency will not fix either. When the problem really is loose skin, and the skin still has some elasticity, the neck can respond well — usually more modestly than the jawline.” — dr. Jesslyn Amelia
Not treated: anyone with a pacemaker, implantable defibrillator or other implanted electrical device; pregnancy. Assessed first: thyroid disease or previous neck surgery; metal implants or permanent filler in the area; active skin disease; keloid tendency; recent isotretinoin; poorly controlled diabetes. Better served by surgery: heavy skin excess with banding.
The neck is also the area people forget when applying sunscreen, and it photo-ages visibly. Daily sun protection supports any collagen treatment (American Academy of Dermatology).

Can XERF tighten a loose neck?
It can improve mild-to-moderate neck skin laxity. It does not treat muscle bands or significant fat, and it is not a substitute for a neck lift.
Is XERF or HIFU better for the neck?
A 2025 meta-analysis found no significant difference between the two for the neck (Modena 2025). The choice depends on your tissue and on the practitioner.
What about horizontal “tech neck” lines?
They are partly skin quality and partly movement, and often respond better to skin boosters than to tightening.
Is the neck more painful to treat?
Neck skin is more sensitive, which is one reason lower energy is used there.
