RF vs HIFU vs Thermage-Type Devices: How Dermatologists Think About Energy-Based Skin Tightening

One randomised trial has compared them directly, and it found no winner. What decides the result is the match between the tissue problem and the treatment — and who sets the parameters.

Short answer. Radiofrequency heats tissue with electrical current, spreading heat through a volume of skin and the layers beneath it. High-intensity focused ultrasound concentrates sound energy into small points at set depths. Both aim to tighten collagen and stimulate new collagen. In a randomised split-face trial, monopolar RF and microfocused ultrasound showed no statistical difference in laxity improvement, satisfaction or side effects (Alhaddad 2019). The choice depends on anatomy, skin, history and the practitioner, not on a single “best” technology.

The two families, briefly

Monopolar RF (Thermage FLX, XERF)Microfocused / focused ultrasound (Ultherapy, HIFU devices)
EnergyElectrical current converted to heat as it passes through tissueUltrasound focused to small points at chosen depths
Heating patternVolumetric: a broad zoneDiscrete coagulation points in lines
Depth controlFrequency, tip and settings; dual frequency is intended to vary depthTransducer depth (for example 1.5, 3.0, 4.5 mm); some devices image tissue during treatment
Surface coolingBuilt into the tipNot typically built in
Regulatory exampleThermage FLX (K170758); XERF cleared as substantially equivalent (K251327)Ultherapy is FDA-cleared; many generic HIFU devices are not
EvidenceDecades of monopolar RF literature; XERF-specific evidence early and smallSeveral systematic reviews (Contini 2023, Haykal 2025)

Thermage FLX works at 6.78 MHz; XERF combines 6.78 with 2 MHz (FDA 510(k) summary K251327). The specific brand comparison sits on XERF vs Thermage and XERF vs HIFU.

What the head-to-head evidence shows

Neither technology has been shown to be clearly superior. Anyone who tells you otherwise is stating a preference, not evidence.

How the choice is actually made

1. What is causing the sag?

Energy devices treat laxity. They do not replace lost bone or volume, remove significant fat or relax muscle bands. If one of those is the main driver, neither device is the answer — see jowls and the neck.

2. How much laxity, and how much fat?

Both work best for mild-to-moderate laxity. A systematic review of microfocused ultrasound lists excessive laxity and a BMI above 30 as relative contraindications (Contini 2023); for radiofrequency, results are better in younger patients with mild-to-moderate laxity (O’Connor 2021).

3. Skin thickness and facial fullness

Clinical observation — not published evidence

Very slim faces with little fat are approached cautiously with any deep-heating device, because there is less tissue to buffer the heat. Volumetric RF with surface cooling and adjustable depth can be easier to tailor in these faces. This is a clinical judgement rather than a published rule.

4. Previous treatments

Areas previously treated with HIFU or liposuction dissipate heat differently and need reduced energy. Recent filler and threads also change the plan (Seoul training notes).

5. Comfort and downtime

Both usually allow a return to normal activities the same day. Comfort differs between devices and between people; RF devices with integrated cooling are often described as more tolerable, but comparative pain data vary by device and are not conclusive.

6. Safety profile and the operator

Monopolar RF: rare burns and small areas of fat loss (Fitzpatrick 2003, Weiss 2006). Microfocused ultrasound: rare nerve injury, fat atrophy and hyperpigmentation, linked to settings and operator skill (Sathaworawong 2018), with serious adverse effects in about 2% in one systematic review (Contini 2023). In both cases the person choosing the settings matters as much as the machine.

7. Is the device genuine?

Generic HIFU machines vary widely. In Indonesia you can check whether an imported device holds a Ministry of Health distribution permit at infoalkes.kemkes.go.id — see how to choose a clinic.

Clinical observation — not published evidence

“Patients often ask me which device is best. I understand why, but the more useful question is which layer of this face is the problem. When the answer is skin and soft-tissue laxity, I have good options. When it is fat, bone or muscle, the honest answer is a different treatment.” — dr. Jesslyn Amelia

Can RF and HIFU be combined?

Some practitioners combine or stage them, but there is little controlled evidence that combining outperforms either alone, and previously treated tissue needs more cautious settings. It should be a planned decision rather than an add-on.

Questions we are asked

Which is better, RF or HIFU?

In the one randomised split-face comparison, neither was statistically superior (Alhaddad 2019). Suitability depends on your anatomy and history.

Is HIFU the same as Ultherapy?

Ultherapy is a specific FDA-cleared microfocused ultrasound device with imaging. “HIFU” is also used for many other devices of varying quality.

What is the best skin tightening treatment in Bali?

There is no single best treatment. There is a best match for your tissue, delivered by a qualified practitioner using a genuine, registered device.

Which lasts longer?

Neither has robust long-term comparative data.
